emr4MD is used in the primary care practice setting not only to document a patient's clinical encounter but to assist in the management of the patient's preventative health maintenance plan as well as implementation of specific care plans for patients with a clinical diagnosis such as, but not limited to CAD, HTN, DM,CHOL.
Our unique Health Maintenance and Disease Management engine is based on the recommended national CDC standards and allows for customizable benchmarks based on clinical diagnosis for a particular patient. This engine assists the physician to proactively monitor the patient's underling problems and to develop the most effective care plan for the patient.
Auto alerts and reminders are generated based on a specific patient's Health and Disease Management protocol and are sent directly to the physician's dash board, notifying of an upcoming or overdue labs, tests, procedures or referrals due for a patient.
